If you are coming here via metro transit the bus routes are:

  • La Clinica: 68 & 71 drop you off right in front of our building
  • East Side Family Clinic: 74 will take you to 7th St E & Mendota St.

We also have a transportation service for uninsured patients.

Most insurance plans are accepted including Medical Assistance, Medicare, MinnesotaCare, HealthPartners, UCare, BlueCrossBlueShield/BluePlus, Medica Choice, Metropolitan Health Plan, PreferredOne and other private insurance.

If you don’t have insurance we do offer a sliding fee. To apply to the sliding fee you need proof of household income (e.g., tax form, last pay stub from your employer within the last 3 month, bank statements, or a written letter from your employer). If you do qualify for our sliding fee discount your payments can range from ($25-$90). If you do not qualify, you will be asked to pay $150 up front and the balance will be billed to you.

Dental offers a sliding fee based on the qualifying discount level and the type of service rendered.

Most pharmacy prescriptions fall under our sliding fee.

Minnesota Community Care has MNsure-certified navigators that are trained experts. We are here to help you fill out your application and enroll in coverage. We can also help with coverage renewals, report changes to your account and provide enrollment follow-up. Services from our MNsure-certified navigators are always free.
